The following documents are required to complete the registration process:
- Registration Form
- Individual Profile & Care Plan Form
- Clinic Information Form
- Visual Media Consent Form
- Copy of Child's Birth Certificate
- Copy of Child's Immunization Records
- Copy of Child's Medical Insurance Card
- Copy of Child's Emirates ID, Passport with Valid Visa
- Copy of Father's Passport with Valid Visa
- Copy of Mother's Passport with Valid Visa
- Copy of Mother's Emirates ID
- Copy of Father's Emirates ID
- 4 Copies of Passport-sized Photographs
- Attached to the Appropriate Forms (child)
- Fees Information Policy
