This application must be submitted with the following:
- A copy of student's birth certificate
- A medical certificate from a local hospital.
- A copy of passport(s) for foreign parent(s)
- A copy of ID & House Registration for Thai parent(s)
- 4 passport size photos of student
- 1 passport size photo of each parent
- Covid 19 Vaccination Certificate / Record
