- Birth Certificate
- Australian Citizenship/Visa/Passport (if applicable).
- Letter of Confirmation of Aboriginal and/or Torres Strait Islander Heritage (if applicable)
- Latest school report
- NAPLAN results
- Medicare Card
- Immunisation records
- Custody orders/parenting agreements (if applicable)
- Specialist reports/documents relating to students with a physical disability, social/emotional, sensory or cognitive condition/s (if applicable)
- Medical reports eg. asthma, anaphylaxis, allergies, diabetes (if applicable)
- If Parents born outside of Australia - Parent’s Australian Citizenship Certificate/Visa
