Enrolment Forms

Thank you for applying to St Philip's College. To assist in making the enrolment process as easy as possible, we have a checklist for you to complete. Please download the checklist and submit it, together with all the applicable forms listed below.

Enrolment Application Checklist (please download and complete). ALL of the following applicable forms MUST BE submitted together. Partial submissions WILL NOT be accepted/processed.

The following forms (where applicable) need to be downloaded, completed and returned to the Registrar, Mrs Kim Bateman.

• Application for Enrolment form (To determine which occupation group you belong to, please download the List of Parental Occupation Groups form).
• Bridging Application Form (If applicable - Year 7 only)
• Permission to Obtain Copies of NT School/Dept of Education Records Form OR
• Interstate Student Data Transfer Note - Parent Consent

Copies of the following forms are required to complete the application process. Please submit these with your Enrolment Application Checklist.

• Australian Birth Certificate OR Passport OR Citizenship Certificate OR Overseas Passport + Visa used to enter Australia. You must submit a Birth Certificate with your application - if you do not have a copy, there are several ways you can obtain a copy (Northern Territory Residents)
• Your child's/children most recent school report/s
• Your child's/children most recent NAPLAN results

Please Note:

Students wanting to enrol as boarders at St Philip's College will be required to fill in a series of additional forms pertaining to personal health and boarding house policies.

Because the information in these forms is regarded as confidential, the forms must be obtained from and returned to the Registrar at the College. They will be sent to you on receipt of the application form.
