Application Required Documents & Steps
In order for us to understand your children better, we ask for the following information shown below to be submitted through our online application form. Please note that you may save and return to this application form at any time before submitting it.
- Student Application: basic student information, family details, emergency contact, and educational history completed via online form
- Scanned ID: copy of passport, birth certificate or health insurance card (to certify date of birth)
- Applicant Photo: recent profile photo in jpg format
- Application Fee: ¥31,500
- School Records: report cards and/or transcripts for previous two years (In English or Japanese)
- Student’s Health Record: Parents are to complete online form or upload basic health history and a scanned copied of immunization records
- Physician Health Form: A complete history and physical examination by physician
- Teacher Recommendation Form: Applications for Grade 1 and above are also required to submit their most current school report. Applications for Kindergarten may also need a school record or additional information from the student’s current school.
- Parental Agreement Form: A document outlining the school expectations and procedures
