Admission Application Procedures
Step 1: Apply Online
Complete and submit the online application, along with the $95 nonrefundable application fee. Please note that the application will not be processed until the application fee is received.
Please complete:
- Applicant Profile
- Parent Observation
- Student Questionnaire (for Grades 5-12)

Step 2: Provide Writing Sample (Grades 1-12)
Print the appropriate writing sample sheet below:
- First Grade Applicant Writing Sample
- Second Grade Applicant Writing Sample
- Third Grade Applicant Writing Sample
- Fourth Grade Applicant Writing Sample
- Fifth Grade Applicant Writing Sample
- Sixth - Twelfth Grade Applicant Writing Sample
Complete in student's handwriting.
Return to Cannon School by either method below:

Step 3: Request Teacher Recommendations
Confidential online Teacher Recommendations are to be completed by the appropriate teacher(s) and submitted directly to Cannon School.
- Teacher Recommendation Request (JrK - Grade 1)
- Teacher Recommendation Request (Grades 2-4)
- Grades 5 - 12 English Teacher Recommendation Request
- Grades 5 - 12 Math Teacher Recommendation Request
Step 4: Request Records/Transcripts (Grades 1-12)
Complete and submit the online Records Release Request to authorize your child's current school to send student records to Cannon School.
Step 5: Applicant Interview
A representative from the Admission Office will contact you to set up your applicant interview.
Step 6: Applicant Entrance Evaluation
- JrK - 3: A representative from the Admission Office will contact you to set up your applicant entrance evaluation.
- Grades 4 - 12: Applicants take the Independent School Entrance Exam (ISEE). The fee for the exam is paid directly to the testing company, ERB, at the time of registration. Please visit iseetest site for more information and to register. Parents or guardians may register their child for individual or group testing. In order for Cannon to receive your child's score, please include our Test Site #340878.
