Ursuline Academy

To be considered for admission to Ursuline Academy, 8th grade students must complete this online application form, which includes uploading a student written statement, and have copies of grade school records sent to Ursuline.

Grade School Records

For the Class of next fifth year, admission to Ursuline will be based on sixth through eighth grade performance and elementary school standardized tests. Grade schools should send the following records to Ursuline Academy via mail (attn: Admissions) or via email (admissions@ursulineacademy.org) for any student who is applying to Ursuline.

• Report Cards for Grades 6, 7, and 8 (8th grade report cards after 1st quarter/trimester)
• Copy of Birth Certificate
• Attendance Records (if not included on report cards)
• Discipline Records (if applicable)
• Standardized Test Scores for Grades 6, 7, and 8
• Evaluation Team Report (if applicable)
• Any IEP/SP, 504 Plan or School Accommodation Plan (if applicable)

If the student applying for admission to Ursuline is currently enrolled at a local Catholic grade school, you do not need to complete a release of records form. Your daughter's school will take care of releasing her records to Ursuline.

If the student applying for admission to Ursuline is currently enrolled at a public school or a private non-Catholic school, please print and complete the Release of Records Authorization Form. The completed form should be returned to the office at the student’s grade school no later than November 1, this year so that her records can be sent to Ursuline before December 1.

Applications for the Class of next fifth year are due December 1, this year.

All applicants will be notified of Ursuline's admission decision, along with any scholarship or financial aid offer, on January 13, next year.

Accepted students who wish to enroll in Ursuline's Class of next fifth year will do so on January 27, next year.
