Guardian’s Requirements

• Guardian’s ID (Drivers Licence, etc)
• Proof of Address Current Utility Bill (Power, Gas)
• Proof of Immigration Status (Certificate of Citizenship copy or Passport copy with Visa/Photo Pages)

Child’s requirements

• Birth Certificate
• Proof of Immigration Status (Citizenship Certificate/NZ Birth Certificate or Current Visa/Photo pages from Passport)
• Recent School Report
• Recent Exam Results
• Testimonial in support of your application

For a New Zealand born child you can upload a Birth Certificate in both the Birth Certificate Section and Proof of Immigration Status section.

Once you have submitted the form, you will receive a copy of it via email for your records. The email will also include sections that will need to be signed by the student’s parent or designated caregiver.
