Preschool Admissions Procedure
1: COMPLETE THE APPLICATION REQUIREMENTS
Submit the requirements listed below via email at admissions@lifeacademy.edu.ph
CCF-LA Forms:
- Student Application Form with 2”x2” ID picture (2 pcs.)
- Pastor’s Recommendation Form (for declared
- Parent’s Covenant with CCF-LAI* Christians only)
- Student’s Developmental History Form with 2”x2” ID
- Guidance Counselor’s Form picture taken within 6 months
- Teacher’s Recommendation Form
- New Parent’s Interview Form
- Student Medical Record
Other Requirements:
- Photocopy and original copy of report card from previous school year
- Certificate of Good Moral Character
- Photocopy and original copy of PSA-certified birth certificate
- Photocopy of marriage contract (if applicable)
- Photocopy and original copy of report card from current school year
2: ATTEND THE ONLINE INTERVIEWS
All interviews will be conducted online. The student should pass all interviews before proceeding to Step 3.
- After completing the application requirements, the student will undergo an interview with the Guidance Counselor.
- If the student passes the interview with the Guidance Counselor , he/she will undergo a final interview with the Principal.
- If the student passes the interview with the Vice Principal, he/she will undergo a final interview with the School Principal. This will be conducted together with the parents.
3: PAY THE RESERVATION FEE
Pay the reservation fee of Php 20,000.00 (non-refundable but deductible to the tuition fee) to secure a slot. Further details will be sent once the reservation period starts.
Grade 1 - 12 Admissions Procedure

- Result of PEPT or PVT (if applicable)
- Photocopy and original copy of report card from previous
- Attendance of Bridging/Tutorial Program (if needed) school year
- For Grades 7–11: Submit an autobiography essay that
- Certificate of Good Moral Character highlights your strengths and achievements.
- Photocopy and original copy of PSA-certified birth
- For Grades 9–11: Submit physical exam with drug test certificate result and chest x-ray result (taken within 6 months)
- Photocopy of marriage contract (if applicable)
- For Grades 10–11: NCAE result
- Photocopy and original copy of report card from current
For Non-Filipino Applicants:
- Photo of passport biopage and latest admissions with valid authorized stay
- ACR I-Card (photocopy of the front and back portions)
