Requirments

Secure all the required documents before requesting an appointment. Otherwise, you will neither be accommodated nor advance to the next step.

For Kindergarten 1& Kindergarten 2

• Authenticated Original Report Card (if Applicable).
• Certificate of Good Moral/ Transfer Certificate (If Applicable).
• Authenticated PSA Birth Certificate/Philippine Embassy Report of Birth (Copy).
• Vaccination Record (Baby Book).
• School Entry Health Assessment Certificate from Primary Health Care Corp.
• Valid Student's Philippine Passport (Copy).
• Valid Student's Residence Permit (Copy).
• Valid Student's Hamad Health Card (Copy).
• Four (4) colored photos (2x2) Blue Background.
• Valid/Updated Father's Passport and Residence Permit (Copy).
• Valid/Updated Mother's Passport and Residence Permit (Copy).

For Grade 1 to Grade 10

• Authenticated Original Report Card.
• Certificate of Good Moral/Transfer Certificate.
• Authenticated Transcript of Records (SF10)-DFA Philippines and Philippine Embassy& MOFA Qatar.
• Authenticated PSA Birth Certificate / Philippine Embassy Report of Birth (Copy).

• Valid Student's Philippine Passport (Copy).

• Authenticated Transcript of Records (SF 10) - DFA Philippines and PE Doha & MOFA Qatar.
• Certified True Copy of the Completion Certificate.
• Two (2) Recommendation Letters (Adviser & Guidance Counselor/ Principal).
• PSA Birth Certificate / Report of Bith ( from the Philippine Embassy).
• School Entry Health Assessment Certificate (SEHA) from Primary Health Care Corp.
• Student's Updated Hamad Health Card.
• Student's Updated Passport.
• Student's Updated Residence Permit.
• Four (4) Colored Photos (2x2) Blue Background.
• Father's Updated Passport and Residence Permit.
