Admission Documents

MIS requires that all new students complete these forms:

Indonesian Citizen

• Surat Akte Lahir
• KTP (both parents)
• Kartu Keluarga
• Immunization Records

For transfer students, except for Kindergarten students:

• Surat Pindah, with legal stamp from the local DIKNAS
• Original Report book
• If you are studying in overseas school, please submit your academic records that has been approved by Indonesian Ministry of Education.

Non-indonesian Citizen

• Passport of student and both parents.
• Student’s birth certificate
• A copy of KITAS (Limited Stay Permit) of students and both parents. If the KITAS is in process, a letter from the sponsoring company (employer) is required until the KITAS is issued.
• A copy of immunization record.

For transfer students

Academic Records : A minimum of two(2) years of the most recent school records (where the student is old enough to possess them), showing courses taken and results. School records from non-English speaking schools which are officially translated into English would be preferable.

All original document must be presentable at the time of admission and should valid at least 1 year by the time of enrollment.
